This is a contact form layout with three fixed fields: Name, Email, and Message.

The Submit button has the following options for the Action parameter:

• Add to Mailchimp

• Add to Mailerlite

• Send to Zapier

• Send to Make

• Forward to your email

After the form is submitted, you can either display a success message or take the user to a different page.

Just select the page, and the user will be taken there after successful submission.
